Required Athletic Forms

Westside Athletics has gone paperless!

Athletes and parents will use DragonFly Max to upload physical forms and sign other documents.

Paper submissions will no longer be accepted after August 26, 2024. Athletes cannot participate until all forms are complete in DragonFly and checked by school personnel.

Paper forms on file will remain valid for one year from date on physical. Digital forms will also need to be updated once a year.

Please remember to submit forms needed to play sports at Westside you will have to upload them within your created DragonFly account

 

The Cheerleading Registration Sheet (your name, parent/guardian information, address, sizes, etc) will be given after the team has been selected.
